What to Bring with You on Admission

Families are encouraged to decorate the resident's living area with personal belongings such as flowers, pictures, and other memorabilia. In addition, please bring:

  • Personal clothing (day and night-time wear)
  • Comfortable, non-skid shoes
  • Lightweight sweaters
  • Personal equipment (wheelchairs, walkers, canes, etc.)

Residents are admitted upon a physician's recommendation without regard to race, gender, religion or national origin.

Admission Information

We recommend a personal visit to Park Manor. Our Admission Director will meet with you to discuss your specific situation and provide you and your family members with a tour of our facility. Sometimes residents are admitted directly from a hospital stay, referred by their physician or case manager. In those cases, it is best to contact the facility to learn about the services and the admissions process.

Park Manor accepts residents 24 hours a day, 7 days a week. Prior to admission, our clinical team will review medical records in order to determine if a prospective resident is eligible for admission and to ensure that we can meet their medical needs. Physician orders are required for admissions to a skilled nursing facility.

Once medically and financially accepted, our Admission Director will contact the responsible party (family member), to schedule an appointment to sign the admission paperwork. This process takes about 1 hour and must be completed before the new resident arrives at Park Manor. It is recommended that you bring the following items with you for admissions:

  • Social Security Card (for photocopying)
  • Medicare Card (for photocopying)
  • Medicaid Card (if applicable)
  • Insurance Cards (if applicable)
  • Power of Attorney
  • Advanced Directive or Living Will
  • Photo I.D.

A physician will be assigned to follow the resident's care and a conference will be scheduled with the resident's responsible party (usually a family member) to answer questions and sign appropriate documents.
